The target audience for this project can be classified as beneficiaries and health care personnel. The health care personnel include medical officers, ANMs, and ASHAs.
This project aims to bring in systematic and sustainable improvement in outcomes from the service delivery perspective, which result in better adherence to GDM care by the beneficiary.
The adherence of beneficiaries to GDM care pathways can be achieved through:
• Improving awareness of the importance of screening and management of GDM
• Facilitating uninterrupted patient monitoring
• Strengthening feedback loop
• Targeted outreach for testing of outliers
• Targeted outreach to GDM positives
The improved compliance of health care personnel with the best practices can be achieved through:
• Point of care alerts on best practices for GDM screening and Management
• Expanded counseling abilities for outliers
• Improved ability to plan ANC sessions
• Improved ability to batch and match GDM positives
• Instant access to target patient group lists
The proposed solution should have the following components to achieve the above outcomes.
From the beneficiary perspective:
• Mobile first ‘Facilitated Patient Monitoring’ (FPM) system for both smartphone and feature phone users
• Help desk integration for the patient feedback mechanism
From the healthcare personnel perspective:
• Integrated case-based e-learning modules on GDM
• Integrated patient counseling through the FPM system
• e-learning modules ANM session resource planning
• Dashboards to guide mentoring and intervention activities
The current scope is to develop all these components with seamless integration and train the staff on the day-to-day usage of this system.
